Sometimes peripherally located maxillary sinus cysts are difficult to manage adequately by standard endoscopic transnasal surgery. Caldwell- Luc approach gives better exposure, but its suffering side effects remain the problem. When we combined a middle antrostomy and a limited sized inferior meatal antrostomy, it could improve the approach and avoid some undesirable side effects. Since Oct. 1995 to June 2000, among 327 patients, this combined method is successfully used in all 7 difficult cases with a similar morbidity of endoscopic sinus surgery.This combined method offers a minimal invasive route for total removal of the maxillary sinus cysts, if the patients were selected carefully.
