Hu man stem cell transplantation is a novel treatment under a ctive research to restore lost brain function following a stroke. In this work, we propose a multiparametric framework, based on multimodal image fusion, for stereotactic implantation of human stem cells in br ain tar gets. Five pa-tients unde rwent image -guided sur gical pr ocedure using this technique. Thre e dime nsional T1-w eighted and T2-w eighted magnetic resonance imaging (MRI) were acquired for topog -raphical an d geometr ical ch aracterization of lesions. Affin e registration of 3D-MRI and si ngle-photon e mission tomogr a-phy (SPECT) is used to localize areas with preserved blood flow in the lesion boundaries, considered as the surgical target. Once it has been selected a s ingle-voxel ma gnetic resonance spectroscopy ( MRS) is d eveloped to verif y th e tis sue's bio-chemical c ondition and for in-vivo follow up pur poses. Preoperative stereotactic computed tomog raphy (CT) was performed for sur gical planning. Frame-based compute r assisted procedure was folio wed for targeting the implant site. A 11 patients u nderwent u ncomplicated s urgeries. Pos t op erative MRI showed high consistency between image-guided planning and transplantation site. Serial evaluations during 12 months demonstrated nos urgery-related ad verse eff ects. Mu Itimodal coregistration offers a s uitable appr oach for se lection and stereotactic access of b rain targets for human stem cell transplantation.
