We don't always manage to have procedures in place for every scenario, in fact it would probably be impossible to achieve. It can also be difficult, even with the best of intentions, to ensure that the procedures that we do have are up to date, being implemented and are effective. The human factor also relies on the theory that everyone has read, understood and retained the vital information that may keep them safe from the risk and hazards they are exposed to. Having procedures in place is extremely important, like legislation they're often based on the learnings from past events. They help create consistency of both practice and outcomes. But are we placing too much reliance on procedures and not enough on people being alert, able to identify and make the right decisions when responding appropriately to risks they face. Dynamic risk assessment is the continuous process of identifying hazards, assessing risk, and taking action to eliminate or reduce risk, in an ever-changing environment.
