ACTIVITY ANALYSIS OF HEALTH RECORD SYSTEMS

Medical record is an important shared means of work that should contain accurate account of patient treatment. The information contained in the medical record must be adequate enough to identify the patient, motivation for treatment, support the diagnosis, and accurately document the reasons and result of the treatment. Patient health care Information is necessary to ensure that health care provider who assumes responsibility for the patient has enough information and knowledge to continue treating the patient. The paper aims to understand the activity of medical record keeping and explores a possible framework to analyse and determine how medical record keeping could facilitate patient care by developing knowledge in a hospital ward setting. The paper demonstrates how Activity Theory as a framework can be use to analyse coordinated work activity of medical record keeping supported by electronic health record system.
