REGURGITATION IN THE POST-OPERATIVE PATIENT

Although clinical manifestations of regurgitation may not be immediately obvious, gastroesophageal reflux (GER) has been reported to range from 27-60% in dogs and cats undergoing anesthesia. Although GER may occur relatively frequently, the incidenceof postanesthetic aspiration pneumonia in dogs appears to be quite low. Ovbey et al estimated an overall incidence of post-anesthetic aspiration pneumonia as only 0.17% (from 140,711 cases sedated or anesthetized over 10 years). This value is relativelysimilar to the incidence of aspiration reported in pediatric human patients undergoing general anesthesia (0.10%). Upper airway surgery, neurosurgery, thoracotomy, laparotomy, and endoscopy were associated with an increased odds of aspiration pneumonia in dogs. Regurgitation and administration of hydromorphone at induction were the 2 anesthesia-related events significantly associated with the development of aspiration pneumonia.
