Borderline Resectable Pancreatic Cancer

Patients with borderline resectable pancreatic adenocar-cinoma are best characterized as those with localized cancers that involve the major mesenteric vasculature to a limited extent, in whom potentially curative tumor resection might reasonably be expected, but in whom attempts at resection would likely be compromised by positive surgical margins in the absence of either vascular resection or preop-erative therapy. Although a distinct subgroup of tumors believed to fit this general description was first recognized as early as 2001, the designation "borderline resectable" was first used to characterize these tumors by the National Comprehensive Cancer Network (NCCN) in 2006, and a consensus conference was conducted to standardize workup and treatment of patients with this stage of disease in 2008 the optimal multimodality algorithm has still not yet clearly been established.
