At the start of the antimicrobial era, it seemed that bacterial infections, including surgical site infections (SSIs), would one day be a thing of the past. History has proved that nothing could be further from the truth. Unfortunately, despite all the advances in medical science, both human and veterinary, bacteria of many kinds consistently manage to get one step ahead of even the latest and greatest antimicrobial drugs. Compounding this is the controversy surrounding veterinary use of certain antimicrobials critical to the treatment of infection with such agents in human medicine.1 In the end, trying to beat the bugs is not going to work, unless we can be smarter about the way we manage patients and their infections. Shifting focus from treatmentof infections to prevention - outwitting the bugs, instead of beating them - is one of the keys.
