A novel reconstruction of the TFCC using the ECU half-slip and the in-terference screw for severe DRUJ instability was described. This reconstruction technique represented the real anatomical reattachment of the TFCC to the ulnar fovea, which induced excellent DRUJ stability and clinical results.Chronic and severe instability of the distal radioulnar joint (DRUJ) is a major issue in the wrist field. Usually, ulnar shortening or direct open repair of the triangular fibrocartilage complex are used for the DRUJ instability,1 however, reconstruction of the TFCC or DRUJ may be needed when the DRUJ instability is chronic.2-7 Since 1998, we treated 29 wrists of chronic ulnar detachment of the TFCC by the novel reconstruction technique using half-slip of the extensor carpi ulnaris (ECU) tendon. We described the technique of the reconstruction and examine clinical results of the procedure.
