Since 1996, we performed 59 eases of midface distraction with/without forehead distraction for 55 patients. For 43 of 55 patients, Le Fort III midface distraction was performed. Le Fort IV simultaneous forehead and midface distraction was carried out for 12 patients,Distraction device is internal type that was devised in our unit,Diseases were syndromic craniosynostosis in 37 cases, frontonasal dyspla-sia in 9 cases, various facial cleft with midface hypoplasia in 11 cases and facial trauma in 2 cases. The patient age varied from 6 months to 42 years, mean 14 years. The mean follow-up period was 71 months (6 - 120 months),Satisfactory results were achieved in 54 of 59 distraction cases, Midface distraction was carried out twice in four patients in whom a satisfactory result could not be achieved at the first distraction.Relapse of the distracted midface were minimal, It seems that midface distraction is superior to the conventional osteotomy and bone grafting method Midface distraction can be well applied to the maxillary hypoplasia patient in whom the midface should be advanced more than 10 mm,
