Trachea allotransplantation for long segment stenosis.

For long airway defects there are no autologous donor tissues available to restore the mucosa-lined elastic cartilaginous framework. The only option is to transplant an allogenic trachea. However, the trachea does not have an identifiable vascular pedicle that allows transfer to the neck of the recipient and restoring its perfusion by microvascular anastomosis of its intrinsic vasculature. Revascularisation of an allogenic donor trachea can only be performed after a prefabrication step. The donor trachea is wrapped heterotopically in well vascularised tissues of the recipient, such as in forearm fascia with its radial vascular pedicle.
