Time after time, we see quality failures even though the needs of the process, the needs of the end use are accurately detailed in a quality standard. Why is this? Nine times out of ten the reason is, directly or indirectly, operator error. This is not new information, but human error continues to be the major source of quality failure in our manufacturing operations. Japan spearheaded the drive for error reduction decades ago, achieving previously unheard of standards of product reliability, consistency and quality. The methodology has been widely published and adopted by others with varying degrees of success.
