A quick method to obtain ilial bone graft

The benefit of autogenous bone graft in many orthopedic situations is undisputed. Autogenous bone grafts are useful when dealing with complex fracture situations, delayed or non-unions, attempted arthrodeses, or filling bony defects. Autogenous cancellous bone graft is the "gold standard" when increased osteogenesis is desired. The most frequent sites for cancellous bone harvest are the proximal humerus, proximal tibia, and the ilial wing. In the author's experience the amount of graft obtainable from the ilial wing has been disappointing. The use of an acetabular reamer to obtain ilial cortico/cancellous bone graft has been described in both the human and veterinary literature. The biologic activity of this material was comparable to autogenous cancellous bone and approximately three times the volume was obtained when compared to the traditional curette method.
