Dichotomy Versus Spectrum Concept In Approaching Psychotic Disorders: Are We Ready To Make The Final Decision?

In recent years, the validity of the dichotomy of psychotic disorders has been intensively discussed during the development of DSM-V and ICD-11 and compared to the pros and cons of a spectrum concept. This paper reviews data concerning the symptomatic appearance and long-term course, as well as concerning different neurobiological dimensions. Although there are reasons to support the spectrum concept from a scientific viewpoint, these might not be sufficient evidence to abandon the traditional dichotomous classification, especially if pragmatic reasons are given enough weight. This was the final decision of the respective DSM-V task forces.
