Many patients undergoing treatment for cancer will present conditions associated with reduced survival time and decreased response to therapy. One possible approach is a real-time monitoring of the therapeutic response to mitochondrial (beta)-oxidation. Mitochondrial (beta)-oxidation acts by decreasing carnitine-palmitoyl transferase I and II activities[1] which is reflected by fatty acid metabolism. Previous studies have linked mitochondrial fatty acid beta-oxidation disorders to specific enzyme or transporter deficiencies and acylcarnitine profiles have been used to facilitate the diagnosis of mitochondrial fatty acid (beta)-oxidation disorders in infants.[2] Here we present a method for the high sensitivity quantitation of underivatized acylcarnitines from rat liver extracts using positive mode tandem mass spectrometry without derivatization of the samples.
