Early and long-term results of open surgery and endovasoular repair for aortic arch aneurysm

We reviewed 210 patients who underwent open surgery or endovascular repair for aortic arch aneurysm. The open surgeries comprised total arch replacement via median sternotomy (group M), replacement of the arch and descending aorta via lateral thoracotomy (group L), extensive replacement via median plus lateral thora-cotomy or the clam-shell incision (group E), and total arch replacement with an open stent-graft (group O). Endovascular aortic repair using a single-branched stent-graft was applied only for high-risk patients (group T). The early and long-term results of open surgery for aortic arch aneurysms are satisfactory in all groups. Some of these lesions can be repaired using an Inoue single-branched stent-graft with acceptable clinical outcome.
