Among several distraction devices available in Le Fort III distraction, TFPS is suitable for infants and small children. Z-Z typed internal ones are ideally designed for parallel setting and more preferred among several kinds of internal ones. Although halo external device is actually useful for definitive distraction, the burden for the patients can not be neglected. Proper devices should be applied for appropriate selected cases. Introduction: Le Fort III distraction is a well established procedure for midfacial retrusion in syndromic craniosynostosis. Although we have several kinds of devices available, a few reports concerning about a choice for selection of an appropriate device for proper cases. The authors report the importance of the selection of suitable device in the last ISCFS conference in Salvador. Here we mention our preference of distraction device for Le Fort III using zygoma-zygoma (Z-Z) typed internal device and transfacial pinning system (TFPS) in our syndromic craniosynostosis series for reliability and amenity for patients.
