Asphyxiated neonates commonly have multi-organ failure including hypotension and perfusion deficit. Vasopressin, a vasoconstrictive hormone, is used in adults with refractory shock, but its use in neonates is limited and anecdotal. Anesthetized newborn piglets underwent a standard protocol of hypoxia and reoxygenation which mimics the clinical scenario. Hypoxic piglets recovered upon resuscitation but gradually deteriorated with cardiogenic shock and altered regional perfusion at 2h reoxygenation. Vasopressin (0.005-0.02mg/kg/h) dose-dependently increased blood pressure without affecting cardiac index and regional organ perfusion. Myocardial lactate was decreased at vasopressin low-dose (0.005, 0.01mg/kg/h), but not high-dose (0.02, 0.04mg/kg/h) infusions, suggesting a dose-related cardioprotective effect of vasopressin.
