I had the good fortune to first meet the late Professor Dame Sheila Sherlock when I was her intern in 1968; thus I was exposed to a wide variety of liver disease at a very early stage in my medical career. I believe close to 40 years as a practising hepatologist is a good basis for reflecting on the changes that have taken place since my somewhat 'nerve wracking' experience as a young intern. In those days management of chronic liver disease was nearly always for an acute change in someone with a previously unrecognized, often silent, chronic condition.
