Transplantation allows to replace included teeth on the arcade and to bring osteo-genic potential in the cleft; in order to compensate or at least to stabilize the rapid osteolysis of the previously grafted bone. A 15 year old girl operated on a left cleft lip and palate was addressed to the dental team to transplant ectopic teeth in the cleft palate. Radiographic examinations reveal large zones with lack of bone. A semi-rigid orthodontic setting maintains the teeth. Endodontic treatment with calcium hydroxide was intended at 3 weeks post-operative. At 6 months, cicatrisation and consolidation were obtained with the absence of unfavourable clinical or radiological signs and then orthodontic treatment was achieved. After 2 years no unfavourable clinical or radiographic signs were observed. Even if we continue to evaluate transplantation of teeth on site of facial slit, to determine the advantage/risk of this treatment.
