Isolating Normal Sperm by Nuclear Characteristics with Subsequent Intracytoplasmic Sperm Injection Does Not Seem to Improve Lowered Fecundity Found with Sperm with High DNA Fragmentation Indices

Several studies have demonstrated that sperm with high DNA fragmentation indices results in lower pregnancy rates (PRs) even with in vitro fertilization (IVF) with intracytoplasmic sperm injection (ICSI). One study found that isolating sperm used for ICSI by first identifying sperm with normal nuclear characteristics could markedly improve PRs in previous refractory cases. IVF with ICSI using sperm with normal nuclear morphology was attempted in 13 refractory cases with DFI indices >30% using specially designed microscope that magnifies the sperm -6000x. Only one delivered pregnancy was achieved (7.7% live PR/transfer) does not seem to improve pregnancy outcome in refractory IVF cases with male partners with high DFI scores.
