Over the past decade, positron emission tomographic (PET) brain imaging has evolved from a potent research tool into a clinically viable imaging entity, while remaining a valuable research method. This evolution has primarily occurred because of significant technical advances in the PET scanning devices and the more widespread availability of commercial PET radiopharmacy facilities, which have been facilitated through reimbursement for clinical PET imaging in oncology and for limited neurologic applications. The recent addition of combined PET and computed tomographic (PET/CT) imaging has further improved the diagnostic utility of this imaging modality by acquiring physiologic and anatomic data concurrently. The areas in which PET brain imaging has proven to be of most value include dementia, brain tumors, epilepsy, and movement disorders. In addition, in select circumstances, PET brain imaging has also been shown to be helpful in cerebrovascular disease and head injury cases.
