Introduction: The orientation of the cup component in a total hip replacement (THR) plays not only an important role for the long-term success of the prosthesis, but also for the postoperative activity level of the patient. The conventional technique in which the component is aligned according to predefined inclination and anteversion angles has few distinguished limitations: (i) Precise intraoperative registration of the anatomical coordinate system is the basis for accurate alignment of the component, (ii) no patient-specific kinematic or anatomic information is incorporated, and (iii) the position and orientation of the stem component is unaccounted for. Existing computer-assisted techniques help the surgeon to achieve more accuracy during planning and navigation of cup orientation, but do not guarantee an ideal kinematic patient-specific postoperative result. We propose an image-free computer-assisted total hip replacement system, that contains an intraoperative patient-specific kinematic planning feature for cup orientation.
