Initial clinical results of computer-assisted distraction osteogenesis by Ilizarov's method

Introduction: Ilizarov's method has been successfully applied to limb lengthening, limb reconstruction, limb salvage, fracture management, osteomyelitis, angular deformities, nonunions and malunions, to name a few [1, 2, 3]. We recently proposed a technique for computer-assisted deformity correction of Ilizarov's method using the Taylor Spatial Frame [4, 5]. In this work, we summarize this new technique and present our most recent clinical results. Our technique modifies conventional approaches to deformity correction using the Taylor Spatial Frame in four fundamental ways: 1. The need for the surgeon to preoperatively measure frame parameters is removed. 2. The performed correction is based on the actual location of the frame with respect to the anatomy; any translational or angular problems that occur while mounting the frame are compensated for immediately, thereby potentially removing the need for residual correction. 3. The correction is calculated based on 3D coordinates from CT data rather than measured from radiographs. 4. The rings do not have to mimic the deformity; we essentially integrate the "chronic deformity correction" phase into the "total residual correction" phase.
