Human reliability assessment procedure for non-Human Factors practitioners

The requirement to conduct Human Reliability Analysis (HRA) within high hazard industries is understood but knowing how to operationalise this requirement and conduct it in a proportionate and fit-for-purpose way, is more challenging. Frequently organisations get it wrong. Either they do not carry out a HRA, conduct it on the wrong tasks or go into too much detail and conduct a fuller analysis when it is not needed (e.g. deriving Human Error Probabilities (HEPs) unnecessarily). This paper outlines a procedure and guidance that seeks to simplify the HRA process and ensure that it is focused on the safety-critical tasks where there is a reliance on operator action. The procedure is aimed at non-Human Factors (HF) practitioners, although some basic training on HF principles is likely to be required. Finally, the process was recently successful applied at a chemical facility and this extended abstract uses the learning from this application to showcase the approach.
