The development of the Internet of Things (IoT) enables more ubiquitous, fast, intelligent, and accessible services in the healthcare applications. It provides large-scale connectivity for health devices, patients, doctors, clinical, and nursing staff to facilitate real-time monitoring based on information collected from the miniaturized objects. Also, it ensures the use of innovative and cost-effective technologies to improve the quality of the patient's medical life. However, unattended and unconditional access to the patient's information can disclosure his private information. Then, it can disrupt the functioning of healthcare services. In this paper, we propose a protocol for access control and role delegation in IoT-based smart healthcare systems. The novelty of proposed protocol consists in securing patients medical records against malicious actions based on context, role and trust assessment. To evaluate effectiveness of our proposal, a series of simulations are carried out using NS3 simulator. The obtained results demonstrate the efficiency of our protocol in terms of success rate and the access time.
