Automation has enhanced many of the largest and most successful industries in America, however its permeation into healthcare has been sluggish at best. Nowhere is this more the case than with care transitions, where the vast majority of decisions, processes, and activities are unstandardized, manual, varied, and slow. However, there are lessons to be garnered from both the healthcare and non-healthcare fields that could lead to breakthroughs in care transitions in areas such as patient risk stratification, post-acute care selection, and hospital learning.
