This paper discusses a “drilling construction failure" case involving a Washington state City’s 1300-foot“trenchless” sewer line and manhole trenchless replacement project performed in 2008 and 2009. The paperhighlights the "accident chain" resulting in over 60% of the new sewer line being installed out-of-grade and leadingto expensive cut and fill rework, more than a year's delay in the project's completion, and claims well in excess ofthe original contract price.The paper’s objective is to not to cast blame or point fingers, but instead to encourage future drilling project successby studying the several factors that combined as a recipe for disaster on the subject project.
