On February 1996, a real incidental transient occurred in a thermoelectric co-generation plant in northern Italy, that cuased a water hammer with serious damages to the plant. Human factors and system related causes played an important role in the generation and progress of the sequence of events. This paper concerns the development of a preliminary study of Human reliability Assessment, based on the attempt to derive important data and information from the root cause analysis carrid out with the reason's model and applying, for the prospective reliability assessment, the classical approach THERP (Swain and Guttmann, 1983).
