Some dermal substitutes such as collagen sponge with silicone sheet, acellular human dermal tissue, or frozen human skin are clinically available, but the long-term results have not been necessarily satisfactory when compared with the autologous dermis, may be due to the incomplete structure and nature. We have been studying acellular autologous dermal tissue for treating congenital giant melanocytic nevus (GCMN). GCMN is originated from nevus cells at the dermal tissue, and its most concerning complication is malignant melanoma with the risk of several percent. After the complete removal of the nevus cells from the skin, good dermal substitutes are needed. In the present study, we inactivated (killed) cultured cells and nevus cells in the human nevus tissue by applying high hydrostatic pressure (HHP) and prepared a transplantable autologous dermal substitute with the native extracellular matrix (ECM) structure.
