In the early '90s, the Advanced Tactical Fighter program (later renamed F-22 and more recently F/A-22) challenged the government and contractor safety communities to develop a cost effective rigorous system safety program. A tailored program, using MIL-STD-882 as a basis, was developed to encompass all aspects of the F/A-22 Weapons System, to include airframe and integration of the F119 engines, associated logistical support systems and training systems. Numerous safety initiatives were implemented to provide the necessary tools to effectively manage the safety program.One such initiative to control documentation volume of each hazard worksheet was to establish Safety Threads, I.e. a method of linking related hazards together. However, these relationships become obscured when considering the volume of 2,200+ active records. This paper will review the inception of Safety Threads and explore the lessons learned from an effort to build a graphical map of the database to understand these interrelationships.
