A Generic Fault Tree for the Movement of Program Critical Hardware

A generic fault tree has been developed to provide basic hazard cause information to support risk assessment and abatement. The elements of the tree evolved from an initial checklist, which in turn was based on experience gained from several moves of expensive and sensitive aerospace hardware. The tree addresses the total operation; the hardware, the move activity, and the operating personnel. Specific move elements assessed include the program critical hardware, facilities, handling equipment, containers, procedures and processes, routes and transportation, receiving areas, and the involved personnel. The tree is a ready reference for hazards that are typical of hardware handling and transportation. Its generalized structure provides a framework that may be tailored for a particular move and expanded with the specific details for the application being assessed. The tree gives the analyst the advantage of having detailed information about potential hazard causes while the move planning is still in its formative stages. The use of the tree provides insight on interface issues including those which might require resolution by other parties or outside entities. The paper addresses use of the tree as a tool to facilitate dialogue within the movement team. The tree can also be effective in progress and status discussions with management.
