High consequence accidents such as those seen in the cruise ship industry, are caused by human error, equipment failure, or some combination of these. Today's engineering safeguards have greatly reduced the risks of equipment failure. This leaves human error, and almost certainly a series of human organizational errors, as the most likely, yet most difficult to control, cause of cruise ship and other high-consequence accidents. This paper takes a case study and shows the importance of developing and maintaining "high-reliability" organizational cultures as the key to preventing cruise ship accident.
