PSM and other Human Factor considerations have improved reliability of many operatingfacilities across many different industries. Similarly, significant improvements in health andsafety have also been made across most industries and are reflected in declining injury frequencytrends and declining numbers of incidents, injuries and fatalities. Nevertheless, organizationallearning and safety culture continue to evolve slowly. In this presentation, a model for creating acontinuous learning safety culture from Shared Learning is presented. The model focuses on aneffective method for moving lessons (knowledge) quickly across organizations in an organizedand consistent manner to foster learning to prevent repeat and similar incidents. Furthermore,the model caters for the rapid dissemination of best practices across the organization to generatebusiness improvements and efficiencies. This paper provides a model for Shared Learning(knowledge) and builds upon the Kaizen Process for Shared Learning. The process is ownerdriven and facilitates rapid organized sharing of learning, both internal and external, to theorganization.
