Safety Management through Learning from Experience in the Chemical Industry: Example of a New Incident Analysis Methodology

SOL has demonstrated its theoretical soundness and practical utility in a great variety of contexts. It has shown to be an efficient tool of incident analysis in the chemical and the nuclear industry as well as its applicability in civil aviation. Its underlying conceptualization as an incident analysis method of a problem solving nature characterizes it as a novel instrument which differentiates it from received mainly checklist oriented approaches. The computer assistance of incident analysis processes gives it a special touch. Its versatility appears to be very broad. SOL is presently further developed as an incident analysis method also in the domain of medicine (operation theatres).
