As the Space Shuttle Program ends and lessons learned are reviewed, the Significant Incidents and Close Calls in Human Spaceflight graphic provides context for the successes and failures of this thirty-year effort. The Significant Incidents graphic displays historical accidents and close calls in order to heighten awareness of the risks inherent in human spaceflight. Its purpose is to distribute this information as widely as possible in order to encourage future programs to learn from the lessons of the past, so as to prevent the accidents of the future.
