The Role of System Safety Engineering in Product Safety

What effect can system safety engineering have on product design? The purpose of his paper is an attempt to answer this question in part, by describing a hazard analysis process for an actua product. The product (or system) for this case study includes a diesel engine. The hazard identified in the analysis is described as a fire and/or explosion resulting from refueling the engine. The fire and/or explosion hazard is the result of fuel igniting from flammable vapor dispersing downwind from a fuel vent outlet to an ignition source. The analysis and assessment techniques employed in the case study include fault tree an lysis, engineering calculations, risk mitigation and risk assessment.
