Condemnation of those responsible for the 2010 BP Gulf of Mexico oil spill, the 2005rnBP Texas isomerization unit accident, the 2005 UK Buncefield oil storage depotrnexplosions, and other industrial disasters testify to society?s strong denunciation ofrnfailures that result in human and environmental catastrophes. Failures in complexrninstallations arise from human negligence and complacency as well as equipmentrnmalfunction. The challenge to industrialists and researchers is to improvernpractitioners? understanding of protective systems, and to ensure that learning andrncognitive processes remain above political and legalistic intrigue. In this paper, thernreports of the 2005 BP Texas isomerization unit accident, the 2005 UK Buncefield oilrnstorage depot explosions are used as the basis of testing how protection is managed inrntwo developing country organizations where failures could have potentiallyrndevastating consequences.
