A transition method for five-axis tool path transfer smoothing, a medium, and a numerical control device (8) for a five-axis machine tool. The transition method for five-axis tool path transfer smoothing comprises: acquiring instruction information from a machining file of a workpiece, and parsing the instruction information to acquire a five-axis trajectory during the machining of the workpiece (S11); performing trajectory pre-processing on at least two segments of the five-axis trajectory (S12); constructing a transfer transition curve between two pre-processed adjacent trajectories (S13); and optimizing a tool machining trajectory according to a speed constraint and an interpolation point of the transfer transition curve (S14). Therefore, the machining accuracy of a workpiece can be ensured, and the feeding speed/acceleration/acceleration jerk of a tool is made to be within an allowed range of a system.
