A positioning device of a surgical instrument for hip arthroplasty surgery comprises: - an arched structure (2) having a first (3) and a second (4) end, wherein the first end (3) can be positioned inside a patient; - a gripping element (7) located at said second end (4) of the arched structure (2); - a surgical instrument (8) that can be coupled to the gripping element (7) at a seat (9) made in this gripping element (7); - a positioning and fixing head (12) arranged at the first end of the arched structure (2) and having connection means (13). The connection means (13) of the positioning and fixing head (12) face the gripping element (7) to couple with a stem (10) that can be inserted into a patient's femoral canal.
