A ventilator system includes a piston reciprocatingly movable within a piston chamber in a piston housing mounted between a motor housing and a valve housing. A piston stem extends from the piston and into a receiver bore in the valve housing. A motor in the motor housing is coupled to a ball screw rod carrying a ball screw longitudinally retained by the piston and extending through the piston stem bore, such that rotation of the ball screw rod by the motor will move the piston either away from or toward the valve housing and thus either increasing or decreasing the volume of an air chamber coextensive with the piston chamber between the piston and the valve housing. Check valves provided in the valve housing enable the flow of air either into the air chamber from an air source or out of the air chamber for delivery to a patient.
