Provided are a medical image processing device which improves the visibility of fluorescence, a medical imaging device, a medical observation system, an image processing method, and a program. The medical image processing device comprises: a calculation unit (932) which calculates the intensity ratio between a fluorescent component signal and a reflected light component signal of pixels in a captured image; a determination unit (933) which determines a fluorescent area and a background area in the captured image on the basis of the pixel intensity ratio of the captured image, said ratio being calculated by the calculation unit (932); and a generation unit (934) which generates, on the basis of the determination result determined by the determination unit (933), a fluorescent image by performing image processing with mutually different parameters on a color component signal of pixels positioned in the fluorescent area and a color component signal of pixels positioned in the background area.
