A catheter and guidewire coordinated pushing apparatus (100) comprising: a catheter fixing element (30), fixedly connected to a catheter (10); a guidewire fixing element (34), fixedly connected to a part of a guidewire (11) provided outside of the catheter; a driving component (20); and a rotating component, comprising a first rotating component (22) and a second rotating component (23), the first rotating component being connected between the driving component and the catheter fixing element, and the second rotating component being connected between the driving component and the guidewire fixing element; the driving component being used for driving the first rotating component and the second rotating component to rotate in a reverse direction, thus allowing the guidewire fixing element and the catheter fixing element to respectively drive the guidewire and the catheter to move alternately or to move simultaneously. Also provided is a catheter and guidewire coordinated pushing system, which is capable of precisely pushing the catheter and the guidewire partly running through the catheter, and is structurally simple, compact, and convenient to assemble.
