PROBLEM TO BE SOLVED: To provide an apparatus for evaluating a degree of frailty and showing a guideline when implementing measures for improving the degree of frailty. SOLUTION: A screen requesting input of frailty degree inspection data is displayed on a display means 3, and when the data is input by the input means 2, the frailty degree of a service user is calculated by a processor 1, and the result is displayed on a display screen. The display means 3 has a recording means 4 for recording the frailty degree, the display means 3 displays the time-dependent change of the frailty degree on the screen, the frailty improvement measure recording program is executed by the processor 1, and the data of the frailty improvement measure is executed. When the screen requesting the input of the data is displayed on the display means 3 and the data is input by the input means 2, the recording means 4 records the data together with the frailty degree at that time and does not implement the frailty improvement measures.
