The present invention relates to spontaneous preterm birth proteomics biomarkers, term birth proteomics biomarkers, and methods of their use. In particular, the present invention provides tools for determining whether a pregnant subject is at high risk of preterm birth, as well as for reducing the risk of preterm birth for a pregnant subject. In certain embodiments, a highly predictable test is provided to detect spontaneous preterm birth during the first trimester of pregnancy in a non-pregnant subject, regardless of birth history, in the pregnant subject. Other highly predictable tests are provided to detect spontaneous preterm birth during the first trimester of pregnancy.
