PROBLEM TO BE SOLVED: To improve the safety of a patient during breath holding in radiotherapy. A treatment support device according to an embodiment includes an acquisition unit, a determination unit, a respiration control unit, and an irradiation control unit. The acquisition unit acquires biological information that changes due to the patient's breath holding during radiotherapy accompanied by breath holding. The determination unit determines the state of breathlessness of the patient during the breath-holding based on the acquired biological information. The respiratory control unit guides the patient's respiration according to the condition. The irradiation control unit controls the irradiation of the patient with radiation according to the state.
