Methods and systems are provided for reliable, convenient, self-managing, and cost-effective identification of central venous pressure. Non-invasive methods and devices utilize changes in transmural pressure differences to generate detectable changes in peripheral venous volume for central venous pressure identification. Changes in the transmural pressure differential can be indicated by intravascular or extravascular pressure changes. The system is non-invasive and utilizes optical measurements of venous volume in the presence of transmural pressure differential changes. The relationship between the change in transmural pressure difference and the change in vascular venous volume is combined with anatomical measurements to determine the subject's central venous pressure.
