The upper sheet is abbreviated around the answer part while address zones are provided in the answer part of the middle sheet and of the lower sheet. The upper sheet and middle sheet are separable in common on one edge of the lower sheet to which they are fixed or the upper sheet and middle sheet in each case are fixed with one edge on two facing edges of the lower sheet whereby the middle sheet is covered at least partly by the upper sheet. Fold marks are provided on the sheets. An additional writing zone is provided in the overlapping region of the sheets.
