Pools betting form template - uses punched holes corresponding to boxes on entry form to permit repeated marking of standard entry

A template is used for the completion of entry forms for betting purposes. It is designed esp. for use with permutation type pools entries, in which the same entry has to be marked on successive reprinted forms. The template consists of a plastics plate which has areas of reduced strength over each box of the printed form so that a required entry can be marked out by removing the plastics over each square to be marked, and the form is marked through the pattern of openings so produced.
