The notebook with a pad of tear-off sheets is particularly for prescription forms. The sheet (14) acting as a support for the pad (10) has a second sheet (15) hinged to it forming with it a first cover. Hinging along the opposite edge of the second sheet is a thin sheet (21) of the same dimensions, forming a second cover with the second sheet. A notebook can be accommodated in the second cover, and there can be notches in the sheet edges to allow selection of a particular sheet in the notebook.
