PURPOSE:To simplify the application work of medical fee by remembering only the number of times of medical examination and the memory address of a unit stored with the medical examination that a doctor makes for a patient and accompaying information. CONSTITUTION:The field of the address of the 1st memory unit 1 set in read/write unit 2 is read out and the 2nd AND gate 13 detects the address agreeing with the address information of the 2nd memory unit 3; and at the point in time when the coincidence is detected, information on the number of times of medical examination is transferred from the 1st memory unit 1 to array totalizing circuit 14 and a medical examination item, form number and points are also transferred from the 2nd memory unit to array totalizing circuit 14, which arrays respective medical examination items in the form of the medical fee application detailed statement while totalizing the points of the medical examination. Then, the result is output to input-output unit 5 and typed out.
